Payment

 

I accept a number of insurance plans. Please contact your insurance company prior to the first appointment to make sure our services are covered and whether or not prior authorization is required. It is your responsibility to understand your insurance coverage. You are responsible for procedures and fees that are not covered by your insurance plan.

 

You may also pay me directly. Please contact me by phone to discuss my rates for services (which may be partially reimbursed as “out or network” by some insurance companies).


CPT (procedural codes) that I typically use when billing include:

 

90791 Diagnostic Interview 

90834 Individual psychotherapy

90846 Family Therapy without patient present

90847 Family Therapy with patient present

96101 Psychological Testing

 

Note: When working with children in therapy, I use code 90847 in that I am a family therapy oriented rather than individual therapy oriented psychologist. Please make sure that your insurance company reimburses for this procedural code before treatment begins.

Co-payments and other fees are payable by check or cash and are due at the time of each appointment.

 

Cancellations and Missed Appointments: You may be billed for appointments that are missed or cancelled without 24 hours notice, other than for illness.
